유학용 예방접종 이력 작성 문의

​안녕하십니까. 세명병원 홈페이지를 방문해주셔서 감사합니다. 

 

해당 예방접종 관련 증명서는 저희 병원에서 접종하신 이력이 있으셔야 소견서 작성이 가능합니다.

예방접종을 진행했던 해당 보건소나 병원에서 증빙서류를 요청하셔야 할 것 같습니다.
